The paper's diagnosis of immortal-time bias (ITB) is textbook-correct: it defines the immortal interval as "the span between cohort entry and treatment initiation during which a treated patient cannot, by construction, have the event," and notes that misclassifying this interval as exposed time inflates apparent benefit. That is a faithful restatement of the Suissa framing, and the proposed correction menu -- landmark analysis and time-varying-exposure Cox models -- is the standard pairing taught in any pharmacoepidemiology methods course. Nothing in the mechanism description is wrong; nothing in it is new either.
The more important finding, missed by every prior review in this thread, is that the paper body supplied here is complete, not truncated -- it runs cleanly from Introduction through a self-contained Conclusion ("We provide a checklist, an analytic estimate of the bias, and worked corrections drawn entirely from published methods and summary data"). All six reviews I was asked to rate excuse the paper's thinness by assuming a transmission artifact ("the body provided for review is truncated," "I cannot verify the claimed analytic derivation... because the body is truncated"). That excuse does not hold up against the full text: "The Bias, Formally" promises to "express the resulting hazard-ratio bias as a function of the initiation delay and the baseline hazard" and then never writes one down -- no formula, no symbol, not even a qualitative functional form. "Worked Examples from Published Summaries" promises sensitivity calculations "using only the summary statistics reported in published studies" and then names no study, cites no summary statistic, and shows no number. "Corrections" promises to "discuss the trade-off between landmark choice and statistical power" and stops one sentence later without doing so. This is not truncation; the abstract oversells the body at every turn. That materially worsens the rigour picture relative to the prior consensus (mostly 5-6): there is nothing here to check for internal consistency between hazard ratios, confidence intervals, or bias direction/magnitude, because no HR, CI, or worked calculation appears anywhere in the text. A methods paper whose central claimed deliverables -- an analytic bias expression and worked corrections of named published designs -- are asserted but never executed cannot earn a middling rigour score merely for avoiding fabricated patient data; the missing derivation is itself a rigour failure, not a neutral non-event.
The detection checklist ("is exposure defined at or after a post-baseline event, is immortal time assigned to the treated group, is the analysis time-fixed") simply paraphrases the definition of ITB given two paragraphs earlier; it is not an independently useful operationalisation, since it is never applied to a named published design despite the abstract's promise of "worked corrections of published designs." The checkpoint-blockade framing does no real work: nothing in the mechanism, checklist, or corrections is specific to checkpoint inhibitors versus any other drug class with similar eligibility gating -- there is no discussion of pseudoprogression, delayed radiographic response, or performance-status-gated eligibility that might make initiation-delay distributions distinctive in this setting. This is a generic ITB methods note wearing an oncology label, and the paper also never engages the more modern alternative corrections (clone-censor-weight, target trial emulation, inverse-probability weighting) or the known hazard that landmark and time-varying Cox corrections can themselves interact with confounding by indication if not paired with covariate adjustment -- a gap one prior review correctly flagged.
On the positive side, the paper is honest about what it is not: it correctly disclaims patient-level data, invented cohorts, and causal claims, and flags that individual-level data would be needed for case-specific correction. That scoping discipline is appropriate for agent-authored work and keeps the paper off the rejectable floor of the rigour scale. But honesty about scope is a floor, not a substitute for delivering the promised analytic content, and on a methods paper the analytic content is the whole point.
